Good Neighbor Program Manager
The New York Association on Independent Living (NYAIL) is seeking a Program Manager for a statewide initiative to assist individuals transitioning from institutional settings to the community. The Good Neighbor program will serve people who are in nursing homes but are unable to return to the community because they do not have social supports who can act as a back-up caregiver. This program will work closely with the Open Doors Transition Center and will utilize transition specialists to identify possible participants. The Good Neighbor program is funded by a 1-year grant from the Mother Cabrini Health Foundation. It is designed as a pilot program and if successful continued funding will be sought. NYAIL is a statewide not-for-profit membership organization that works to remove barriers to full community integration of people with disabilities. NYAIL members are Independent Living Centers (ILCs), consumer controlled, cross-disability providers of advocacy, services and supports to people with disabilities.
Responsibilities
The Good Neighbor Program Manager will report to the Open Doors Project Director. The Program Manager will help develop the policies and procedures necessary to start-up this new program. They will work collaboratively with NYAIL Transition Center subcontractors to recruit participants and neighbors who will meet their needs. Responsibilities include, but are not limited to:
- Work with Open Doors Project Director to create procedures, forms, trainings and outreach materials for the Good Neighbor Program.
- Conduct trainings and outreach as needed (statewide travel required).
- Work collaboratively with Open Doors team to identify participants for the Good Neighbor Program
- Work with ILCs to recruit neighbors.
- Work with Open Doors Project Director to modify database for data collection.
- Monitor database for accuracy and run reports as needed.
- Utilize database to document work.
- Monitor ongoing services provided by neighbors to participants.
- Work with Open Doors Project Director to write proposal(s) for ongoing funding.
- Perform other duties as required or assigned.
Qualifications
- Minimum of a Bachelor’s degree.
- Experience running statewide programs and developing program procedures.
- Experience with the concepts of Independent Living, consumer control, and a strong commitment to promote and advocate for the full integration of individuals with disabilities.
- Knowledge of the state’s Medicaid program and community resources that assist people with disabilities in living independently.
- Experience with Nursing Home Transition and Diversion (NHTD), Traumatic Brain Injury (TBI) and OPWDD waivers as well as Medicaid Managed Long Term Care (MLTC) preferred.
- Excellent interpersonal, organizational, computer, and communication skills.
- Personal experience with disability a plus.
This is an Albany-based position. Regional and statewide travel required.
